Write the word on a paper and throw it in the trash! Procrastination is embedded! Sorry it's the simple truth. It's like eye color...your born with it...doesn't mean you can't temporarily change it...but in the end it's still the same...!
Laziness can be changed. Get enough exercize and eat smart and get plenty of sleep!

Get organized, and fill up your schedule in advance. Have a friend or counselor hold you accountable for being lazy and/or putting things off. Hang around highly motivated people who epitomize how you would like to change; this will rub off. If it is a recurring problem, however, then get prescribed Ritalin/Adderall/etc. which will help you get organized and motivated at the same time, thus beating the procrastination and laziness.

My advice would be to build up to being active. Set some goals for yourself every day. It's amazing what exercise can do to help you be more energetic. I used to be a bad procrastinator and I guess somewhat lazy...but I began getting daily exercise and feel better about myself and have more energy than I've had in a very long time.

Everyone procrastinates, it's human nature. The only way to overcome this nature is to constantly fight the urge to put things off til later. Push yourself harder, and discipline yourself more. Try to remind yourself of all the consequences that come with procrastination, and how much harder it will be on you in the end.
