I can give you an example.
I used to take my family to Whataburger weekly.
We went on the same day, same time, and to the same location every week.
Then one day we all got half cooked burgers. Nothing like a bloody patty to turn a stomach or two. When I took them back to the counter, I was very friendly and asked the counter person to please cook the patties again. She actually rolled her eyes at me. This was the first time we ever had a problem.
I calmly went back to my table and waited on the burgers and visited with my family. When they were ready she brought them out. They were burnt, the cheese was black and she didn't even say a word.
We finished our drinks, ate our side items and asked to speak to the manager. She assured us it would never happen again, yet she didn't offer to replace or refund the uneaten burgers.
We left and I called the Head Office for Whataburger (which coincidentally is in the same town) and explained the situation. The receptionist took my message and hung up.
One week later I recieved a call from some head honcho apologizing and telling me he would send my some gift certificates. I gave him my address and ended the call.
It has been almost two years. No gift certificates. Needless to say, no more Whataburger for our family, ever.
I think it is appalling to promise something and not deliver when your company has failed to take care of a customer. It is shameful to have a grown man lie about something so silly.
I recently got into a conversation in a waiting room with several other women and 3 of them said they had stopped going to Whataburger because of the lack of quality food and lack of customer service. One woman had also called HQ and been promised gift certificates and not recieved them years earlier. This speaks to their integrity.
You would think that Whataburger locations in the Company's hometown would be the best locations. Appparently not.

Bad service is at the top, but a close second for me is when the eating establishment gets my specific order wrong. I used to work in a restaurant and understand the hectic atmosphere of a kitchen, but if a customer places a specific order or request, they'll be repeat customers if their request is honored. I have many food allergies and am allergic to many "common" foods. If I request, for example, no onions on my sandwich, I don't want onions on my sandwich. Period. Once is understandable, the second time I stop eating there.

Poor service. If I don't feel valued as a customer at a restaurant, i will never go back again no matter how good the food is.

If I go out for a meal the food has to be good. If the food is great and the service a little slow or the atmosphere a little noisy I might still go, but if the food isn't good, service and atmosphere won't bring me back.

Bad service mainly. If I had bad food but the service was good I'd give the place another chance and try something else. If I got bad food again...forget it. If the food is awesome I'll forgive slow, but not rude, service.

I'ts either because the food is crappy or I found a better place to eat. It is hardly because of price. But the number one reason why I go back is because of good "hot" food and friendly service.

Too loud, server forgets what I want, kitchen screws up order, not enough variety on menu, lack of subsitution flexibility, smoke, do not carry non-alcoholic beer, seat me next to kids, serve some people in my party way earlier than others, forget to tell me they're out of my selection until half the others are eating, too much noise made by staff taking away, setting dishes, forgetting to bring water and then when I remind them they say they will but forget.
