Denim jeans top the list. They are designers' cruel joke upon a gullible public. They are just horrid in looks, but some fashions damage the body. The Edwardian "S line" corsets made women walk like pigeons with backs unnaturally curved. Any corset was bad enough, for it squeezed and displaced bodily organs. In the Roaring 20's women discarded corsets, but they squeezed in breasts, rather than waists. That's still not good. Shoes in many periods have ruined feet. Also in Edwardian days, women wore hobble skirts so tight at the ankles that they could barely shuffle along. How about Chinese foot binding? Europe has no monopoly on appalling fashions. I could list many more, but I don't have time.

In recent history, the 6o's had some horrible sack and chemise dresses that everyone looked horrible in. In the 50's girls tried to see how many crinolines they could get under their skirt. There were actually "50 Yard" crinolines, in other words made of 50 yards of net! I guess this came back from the movie "Gone With the Wind" and everyone wanted to look like Scarlet O'Hara.
The beehive hairdo was horrible. Some girls wouldn't wash their hair, just kept adding more hairspray. Can you imagine? They wrapped their hair in toilet paper at night to keep it from getting messed up.
In the 40's and 50's the Zoot Suit for men was popular. The pants were wide, the more wide the more fashionable, and then they tapered at the ankle. Ugly.
In our metro area, some men are now wearing skirts on a daily basis. They are called Man Skirts and are based on the kilt which look fine in a parade of Scots but weird on a man dressed to go to work and you wonder whether they have underwear on underneath.

Well, we just managed to get out of the low rise jeans + thong phases (seems like the jeans are still around, but at least the thongs won't be hanging out anymore).

Also, pretty much the entire 80's. Off the shoulder tops, leggings (which are supposedly coming back in, ugh ugh ugh), shoulder pads...

Women wearing flip flops with an otherwise classy outfit. Double pluggers just ruin any outfit apart from your swimsuit and sarong, ladies.

Low low rise jeans. They make your *** look weird and girls mid-drifts hang over the tops (even the thin girls)! Ugly!
Acid washed jeans were pretty bad too.
