The narrator (Norton) finds a bunch of books titled I'm Jack's spleen or I'm Jack's heart, educational books for children explaining different body parts. He uses it as a metaphor for the rest of the movie when he wants to describes his feelings, i.e. I'm Jack's raging anger. Jack isn't anybody really.

In the film Fight Club, the real name of the protagonist (Ed Norton’s character) is never revealed. Many believe the reason behind this anonymity is to give "Jack" more of an everyman quality. Do not be deceived. "Jack" is really Calvin from the comic strip Calvin and Hobbes. It’s true. Norton portrays the grown-up version of Calvin, while Brad Pitt plays his imaginary pal, Hobbes, reincarnated as Tyler Durden.

Jack is the character that the narrator reads about in articles about organs in the 3rd person (i am jacks colon ect ect) and he also uses that phrase to refer to himself later in the movie (i am jacks complete lack of surprise) but jack is not the name of the character. and yes, the narrator and tyler are the same person.
