Each state has an open report on all nursing home. try your state home page and go from there.

You can also call the local HRS department and ask for local nursing home report.
They may be able to lead you to the right place for a VA listing.
VA also has social services department in each city, they may or may not give the place to check their rates.

It is the law. The nursing home you are reviewing must provide for you to read their report card. They are supposed to have it available for the asking, so just ask. Its usually a notebook with a whole lot of reading in it from the department of public health.

Department of Inspections and Appeals...that is the agency that inspects Nursing Homes for compliance to rules and investigates any complaints.
