Depends how heavy you are lifting. Ideally for lifting weights you should not use a running shoe. Running shoes tend to have high cushioning in the heel for impacts during running. Cross-trainers are a better option for you since you mix up your workouts. However, if you become more serious in lifting, consider a shoe similar to what wrestlers wear or one with low cushioning to avoid unnecessary strain on the ankles.

To tell when to change out your shoes... look at the heel from the side. If the side-wall is heavily creased the impact cushioning may be worn out. Also, look at the tread to see if it is worn out. If so, another indicator you need to replace them.

Depends on several factors: your weight, what type of cardio (an elliptical will be easier on your shoes than running on a treadmill or doing aerobics), and the quality of the shoes probably being the biggest factors.

Basically you should get them changed out at least every 6-8 months. However, since you wrok out so much get them changed every 6 months to ensure you keep the durability in your shoes to prevent possible injuries. Shoe are very important in exercise and can make a huge difference in performance.

best is to have at least 2 pair, or more, and rotate them every few days
remove insoles after work-out, to dry the insides of shoes
don't leave them in the trunk of your car,,too dry and hot or too moist depending on where you live
perhaps one pair for weights and another for cardio,

Every 2-3 months.
You want to make sure you have well operating shoes, especially due to the fact you work out as much as you do :)
