No, my tongue piercing didn't hurt at all... it was more of a weird feeling than pain. What came after, though, is what hurt.
First, you stick your tongue out, and hold it there, so it could be marked for piercing. Then they hold your tongue firmly with clamps (not so much that it hurts, but enough to hold it still). All the while, your tongue is getting pretty tired (stick it out for two or three minutes, and you'll see what I mean).
All you feel at first is a little prick, as the needle pierces the outer layer of skin. Then, a weird pressure as it passes through your tongue... it doesn't hurt, but it's truly a unique feeling. You get a sense of how thick your tongue really is. Then once the needle passes through, the guidewire for the jewelry passes through your tongue. If you've ever had a long hair on your eyeball, and as you pull it off, you can feel it drag across your eye... you know the feeling of what the guidewire passing through your tongue feels like already. With a little bit of tugging and fiddling, your jewelry is inserted.
Your tongue will feel VERY tired, and awkwardly heavy, but nothing too bothersome for the next few hours.
The real pain doesn't come until later that night, when your tongue starts to swell up. Imagine the ache of running a marathon, at the speed of a sprint... and all of it in your tongue. Plus, your tongue is considerably swollen. Don't try to eat anything solid, spicy, acidic, too hot, or similiar extremes. It'll just inflamme the pain. To care for it, eat lots of cold foods (ice cream FTW), chew on ice chips, drink ice water, and take an anti-inflammitory medicine (like Advil or ibuprofen).
During this time, don't be surprised if you talk with a lisp. It'll pass when the swelling is gone, and your barbell is properly resized a week later.
It doesn't start fading away until the next day, and the swelling decreases a little bit. That 2nd day is a little more tolerable, decreased to a constant, dull, throbbing feeling. Every once in a while, it'll flare up to 1st day pain, though.
The 3rd day is when the pain subsides to next to nothing, and only every once in a great while is there a flareup to a 2nd day pain. However, your tongue is no longer swollen, and your barbell is now too long. Biting down on it happens alot if you're not careful, and that hurts. Alot.
4th day, no pain at all (aside from the side effect of biting down on the barbell).
A week passes, and you go get your barbell resized (so you stop biting on it ^_^), and by having it removed, and a new one reinserted aggrivates the wound, causing pain to rise back up to 2nd day levels a few hours after the reinsertion... but by that night, the pain should subside to 3rd day levels... and the next day, it'll be as if nothing happened.
