The Health Insurance Portability and Accountability Act of 1996 (HIPAA) requires that your doctor keep all of your records private. However, some vendors (software suppliers) could still have access that requires them to maintain their systems. Your doctor is required, upon request, to provide you with a list of anyone who could possibly be allowed to see your data and what procedures are in place to insure that other people cannot see your data.

Your doctor is required by law to keep your health records confidential. This is mandated by HIPPA and any violations can result in serious consequences.

Your doctor has to automatically keep anything you say confidential, you do not have to sign anything. unless child protection is involved, or you say you are going to harm yourself or others.
