Excaliber is both the sword in the stone and given by the lady in the lake. Uther Pendragon (Arthurs father) is the one that put it in the stone before he died. Arthur during a battle had "broken that which could not be broken". It was excaliber, and he threw the peices into the river. Not long after the lady of the lake (one of them actually) had returned it to him whole again, when he returned to the spot where he tossed it.

In surviving accounts of Arthur, there are two originally separate legends about the sword's origin. The first is the "Sword in the Stone" legend, originally appearing in Robert de Boron's poem Merlin, in which Excalibur can only be drawn from the stone by Arthur, the rightful king. The second comes from the later Post-Vulgate Suite du Merlin, which was taken up by Sir Thomas Malory. Here, Arthur receives Excalibur from the Lady of the Lake after breaking his first sword in a fight with King Pellinore. The Lady of the Lake calls the sword "Excalibur, that is as to say as Cut-steel."

Excalibur was given not to Arthur but to Merlin and yes it is the same sword that was in the stone. Arthur was not the first to have the sword. It was the sword of kings.

His father received it from the lady in the lake, when mortally wounded he incrusted it in the stone, from which Arthur recovered it. When Arthur is dying he sends Sir Gallahad to return the sword to the lake.

At first Excalibur was given to Merlin, who then gave it to Arthur's father....Arthur's father stuck it in the stone, and Arthur pulled it out and became king....When Arthur beat Lancelot, it broke and the Lady of the Lake fixed it and gave it back to him....so technecally, it's both.....weird huh?
