Depends on where you are. Go to a courthouse and ask. Usually in the US, you file an action called "Petition for Change of Name"; you get to put an ad in the legal notices of a newspaper, and eventually go before a judge. If no one objects (and almost never does anyone do so), the judge will issue a writ ordering the change to take place. Then you get to write to the DMV, the Registrar of Voters, your bank, all of your creditors, and anybody else who needs to know. You'll need a new passport, too.

Go to the courthouse, get the paperwork to fill out, fill it out and take it back with fee. You have to publish it in a local paper for (I thihnk) 3 weeks? Might be two before they will grant it. Fee around here is around $300.
