I am looking for a tenent intent to move out form.

You don't need a form. Just a simple written note will do nicely.

Dear Landlord;

I will move of my home on (insert date here) when the lease expires. I will turn the keys over to you at that time. Please advise a convenient time on that date for our final inspection.

Mail it certified mail, return receipt requested and keep a copy for yourself.

You dont really need a form, you can just do a written letter with the date on it telling them when you plan to have it vacated by. That's waht i've always done.
