Excellent question. I assume you are referring to Clinton's missed opportunity to get Osama and Bush totally screwing everything up.

Missed Opportunities are situations that pass by and not noticed until to late. Usually not malicious or intentional but sometimes people get hurt. Catastrophic Missteps are usually due to ignorance and poor judgment. Oh let me throw in arrogance too! This can be done maliciously/intentionally to set off a chain reaction to other events and effect the outcome. Or, it can be a single event that creates kaos. Sound familiar? yes,, the pious fiscal conservatives spent 5 years and millions of taxpayers dollars in an effort to bring down an American president anyway they could, finally settling on a sexual tryst and exposing our nations children to the detailed explicits of a White House intern whore and her sperm stained dress, ignoring our nations security,, shame on them all for allowing the world to view the USA as the x-rated illiterate immoral of the century,,, then to go on to back a presidents invasion of a country for another 5 years,, based on masterful deceptions and lies,, expending our treasure, security and our military might for ill-gotten gain,, holding Americans hostage to fear and religious radical extremist hostility,, shame on you all who share in this twisted behavior and the way you see of our beautiful otherwise peaceful and glorious world,,,,

"Catastrophic misssteps" occur when something disasterous was done. (like the Iraq war)
