The most popular hotel ratings are based on two companies... Mobil which rates properties in Stars, and AAA which rates properties in Diamonds. Both of these companies publish travel guides and use their annual ratings of properties accross the world to help members distinguish between low and high end properties.
Here are guidelines from AAA's site:
One Diamond
These establishments typically appeal to the budget-minded traveler. They provide essential, no-frills accommodations. They meet the basic requirements pertaining to comfort, cleanliness, and hospitality.
Two Diamond
These establishments appeal to the traveler seeking more than the basic accommodations. There are modest enhancements to the overall physical attributes, design elements, and amenities of the facility typically at a moderate price.
Three Diamond
These establishments appeal to the traveler with comprehensive needs. Properties are multifaceted with a distinguished style, including marked upgrades in the quality of physical attributes, amenities, and level of comfert provided.
Four Diamond
These establishments are upscale in all areas. Accommodations are progressively more refined and stylish. The physical attributes reflect an obvious enhanced level of quality throughout. The fundamental hallmarks at this level include an extensive array of amenities combined with a high degree of hospitality, service, and attention to detail.
Five Diamond
These establishments reflect the characteristics of the ultimate in luxury and sophistication. Accommodations are first class. The physical attributes are extraordinary in every manner. The fundamental hallmarks at this level are to meticulously serve and exceed all guest expectations while maintaining an impeccable standard of excellence. Many personalized services and amenities enhance an unmatched level of comfort.
The following is based on personal experience:
One thing to keep in mind is that a hotel can have everything it needs to be a 5 star or 5 diamond property but unless it has great customer service then it will never get past a 3 star rating! The hotels and properties that get a four star or 4 diamond rating (or higher) excell in customer service!
Now, the highest level of a rating is either 5 stars or 5 diamonds. The difference between a four diamond or star and a 5 rated hotel is AGAIN the level of customer service. At a 5 star/diamond property you are pretty much treated like royalty and well taken care of. For instance, things I remember from the AAA guidelines are that at a five star property you personally taken to your room by a host, they leave a gift (like chocolate or ?) on your pillow, and you receive a personal note from management thanking you for your stay. Of course there is a lot more.
Another thing I remember reading is that based on the high level of customer service that is required of 5 star/diamond properties, the sizes of the hotels are generally small... around 200 rooms or less.
Now, if you want to put all of this into context lets use Las Vegas as an example. For the longest time (long before hotel ratings ie customer service became a factor) Caesars Palace and the Golden Nugget downtown were the most consistent 4 star AND 4 diamond properties. Their level of customer service was top notch. The only thing preventing them from getting a 5 star rating was that they were limited by their sheer size and numerous rooms. For a long time, other properties found it very hard to go from a 3 rating to a 4 rating. Remember, the 3 to 4 rating is the division where customer service comes in. Properties such as the MGM Grand found this out. In my limited research, they got a 4 star rating one year by Mobil but were still rated by AAA as a three diamond property. The following year, Mobil ranked them back to a 3 star property.
Steve Wynns properties at the time The Mirage, Treasure Island consistently sold their properties at higher than what they were rated. Steve Wynn really wanted a 5 star/diamond property. Well, shortly after the Bellagio opened I heard it was rated 5 diamonds. I honestly thought this was wrong, based on everything I learned about hotel ratings. But sure enough, I looked into it and found out that AAA sold out their long prestige of hotel ratings and gave The Belaggio a 5 diamond rating. Maybe Mobil did too?
For the record, I believe the first 5 diamond rating in Las Vegas went to The Four Seasons which occupies the top floors of the Mandalay Bay. This really fit everything I have learned about 5 star/diamond hotel ratings. The hotel needs to be small so that the utmost of customer service can be given to each guest.

There is really no way to answer this. It would depend on so many factors - what percent of their rooms are occupied, how many events they host, what they charge for rooms, what their overhead costs are, etc. There are 5-star hotels which are quite profitable, and there are 5-star hotels which don't do as well. Being a 5-star hotel does not guarantee high profits, and on the other hand being a 3-star hotel doesn't mean they don't make money.

stars are given to hotel if they have satisfied the facilities required for a basic standard procedure. A five star hotel is given 5 stars because it have all the attributes and facilities a five star would have.

you can see most of the great hotel just got 4 star, the reason is they don't have swimming pool.

well while deciding the name of the hotel the owners take a walk in the country side. they reflect alot and after they have reached the state of an enlightnment they see a magic fairy who give them five options from which the owners have to choose one. Based on the options some choose RITZ , others TAJ, and still others that get the option choose FIVE STAR.
