As far as outline form, a 5 paragraph essay consists of:

First Paragraph is generally an introduction to what the problem is and what you are going to try and cover.

Second paragraph is the most important statement to support your answer but expanded from the 1st paragraph.

Third paragraph is the next most important topic expanded from the 1st paragraph.

The fourth is the last supporting statement, again it is expanded from the 1st paragraph.

And finally, the last paragraph is the conclusion which will restate what you wanted to cover and your 3 supporting statements.
