Well, for starters, make a fist when putting your hand thru to the bottom of the legs because whether you have nails or no nails, the chance of rips and tears start from the nylon catching on the little pieces of stray nail or skin on your finger tips. Make sure you have the correct size for your body. The way I finally have to do it is to wear thigh highs with a garter or the type that just stay up on their own beacuse my legs are thin and I have a tummy, so the size at the waist fit but the legs were too big. When you put your foot into them, make sure to bring the hosery all the way to the bottom to where you start rolling them up and make sure your toe nails are neatly trimmed and curl up your toes so to not cause a tear in the bottom. If you get a little tear somewhere, anywhere in your hose and you catch in time, paint it with clear nail polish and it wont run any longer.

It could be any combination of the pantyhose being too small, possibly a rough nail or sharp ring or something. Maybe callouses on your heels or toes might catch.
Try a size larger, perhaps thicker, too. If you tend to poke through with your nails, you can get cotton gloves to wear which will prevent this. Taking care of your feet will eliminate them from being the culprit.

make sure your nails are not rough and if your hands are dry use lotion on them before you put them on. Use patience when putting them on, this cannot be rushed very well. If you get a snag, use clear nail polish on the run. Just a little works.
I wear thigh highs which are easier to wear and if one runs and you have another pair the same color you still have an extra good one. They are more comfortable too.

because they are made so cheap. Probably in china! I put 2 pair on last week and they got a run as soon as I put them on. At 4.00
a pair that is hard to take. I think all women should refuse to wear
the darn things.

Sometimes buying a bigger size helps, so you don't have to pull them so tight. But they're really only intended to last a couple wears anyway, unless you buy the really pricy kind, which can run anyway.
A couple alternatives that will last longer (but not forever!): fishnets or tights.

Make sure your fingers aren't rough, and you don't have any hangnails. Some people put on gloves before putting on their nylons. I have heard keeping them in the freezer keeps them from running so easily but I don't know that from experience.

sometimes they will get caught on miniscule uneven edges of your toenails. i always bunch my toes under before putting them on, it seems to help. also, roll them up, don't pull them. have the whole leg side in your hands and out your foot in, toes bunched, then roll it up your leg. if you try to stretch it on it will tear easier.
