Transcribing errors. When a cell reproduces, it must first copy all of the genes. It does this with an polymerase that unzips the DNA strand, reads it, and "calls" for the necessary nucleotides.

If the polymerase makes an error, the DNA is not correctly reproduced. If the cell in question is a reproductive cell (ovum or sperm), that error will be passed on to the offspring.

Sometimes the error is minor and does not cause problems. For example, there are something like ten different versions of hemoglobin, caused by different geners in people, and they all can carry oxygen well enough.

Sometimes the error is major and results in an non-viable fetus, a sport, or a version of the creature is less well adapted and does not survive to reproduce.

Sometimes the error results in a creature that is better adapted and more likely to reproduce. And slowly, over time, an amoeba evovles into a human.

Why assume it was an error? It could have been a random mutation (Darwin), or an intentional intervention (God, or superior alien). Perhaps the error was leaving so much of the animal, emotional brain intact.
