Javed Miandad against India, in ODI. Bowler was Chetan Sharma. In school boys cricket against England, playing for India Surinder Amarnath hit 2 sixes of last two balls to win the match.

It´s Brendan Taylor from Zimbabwe. He did it against Bangladesh in 3rd ODI in 2006.

And in First-Class-Cricket only once has happened (as far as I know), that too recently in a match between East-Zone and Central-Zone. Ganguly was the bowler (East-Zone) who bowled the last over and was hit for six by Dishant Yagnik. Not only he sealed the match, but also Ganguly´s career. He was thrown out of the team and it was up to him to perform in the Domestic level to make way for himself for the national team. But he didn´t, he couldn´t even defend six runs on the last ball. So, to an extent it was end of Ganguly.
