There have been two space shuttle accidents to date, Challenger in 1986 and Columbia in 2003. In both cases, there were seven crew members on board, and they all died, making a total of 14 people dead in space shuttle accidents. The only other people who have ever died on board a spaceship were the three crew members killed in the mercury fire, so the total roster of people dead in spaceships is 17.

There were two shuttle disasters.
To correct "green_meklar" It was the Apollo 1 fire on the launch pad that killed 3 astronauts, not Mercury (a one passenger vehicle). Although one of the astronauts who was killed was Gus Grissom who was one of the original Mercury astronauts.
