I believe the thumb, index, ring and pinky all thought about their situation eons ago. While each was important in their own way, they knew that the lines of communication broke down when expressing anger and dismay. While making a fist and soundly beating upon someone was a normal response, it sometimes didn't drive home the point the holder of the hand was initially trying to make.

Pointing the index finger was already taken to mean "hey...you...over there" or pointing out a specific location or object.

The ring finger....well, women usurped the ring finger for their own personal gain.

Now for the pinky. The pinky has evolved from a measure of useful grasping, and is still used as such, but now has been demoted to a measure of varying status. Examples would be for people who have attended finishing school and learned the proper way on how to sip soup or the way flamboyant homosexuals use it to emphasize their communication.

The thumb is what sets the human race from every other species. While there are others that have thumbs, humans have utilized it for its physical characteristics, its communication skills and its many other properties. Someone can let you know how they're feeling by a thumbs up or down, or even if they are in a so-so mood. It allows us to pinch, torque, grab to varying degrees and finally, it allows us to relax...especially babies and toddlers...or even low functioning adults...this is done by sucking on the thumb.

This all leads us to the inception of the middle finger. All of the metacarpals agreed that each provided a serviceable functionality that could be utilized in many situations, but all also agreed that they didn't have that one peice that gave it specific representation. While the thumb was considered a part of the hand, it was like a deformed friend, while cool and had very special talents, just wasn't really a finger. And finally, they desired someone who was logistically centered and could represent the collective in one fell swoop. Someone who could tell others, "hey, f-off", or, "this is what I think of you and this situation". From this point, the middle finger was born. The index and ring fingers said their final words, side-stepped and made new ground for their future partner. After eons of evolution, the middle finger has become the pinnacle of communication, the balance of strength and the leader of the hand and in all facets of life. In unision, the hand is a much more powerful weapon and inventor as well as manipulator of the environment due to the addition of the middle finger.

And this, my friend, is how and why the middle finger was created.

This was created years ago and it relates to archery. When they still used crossbows and longbows. If you were an archer (correct name?) and you were captured they would cut off the fingers you used for pulling back the string on the crossbow . So when people escaped capture and they still had their fingers, they used a middle finger or two-fingered gesture (known as 'the vikki'-for some reason, unknown to me-in Glasgow, where I live) to show that they still had the relevant fingers for archery use.
