You need to amend the birth certificate, but you might also have to do a legal change of name. If that's the case, you can only do it with the father's consent since the child is not above age 18.

The child should have the fathers last name because it was the sperm that got u pregnant.
To change a birth certificate , u would have to go to court.

u need to file for a new one in the county/state baby was born it will cost money but well worth in long run for your childs sake...
