The Green Mile and The Shawshank Redemption. Both followed the books to a T. Two perfect adaptations.

"The Bridges of Madison County". I usually think the book is better than the movie, but in this case, the movie is much better.
Essentially books are much longer and can by character development and other means convey the author's meaning. In the case of Bridges, the book did little more than fill in a plot line with characters who were pasteboard thin. The movie on the other hand had the excellent performances of Meryl Streep and Clint Eastwood which supplied the deficit of the book.

Hitch Hiker's Guide to the Galaxy was a wonderful book, and a wonderful movie, but a gawed aweful movie based on a book.
In regards to faithfulness to the text and a good see, I nominate "Holes", but that also has attached a huge childhood bias. I worshipped the book, along with the Narnia series, which would be my second nomination.
I'm surprised nobody's mentioned Harry Potter! :-P

A few...
The Princess Bride, because the author of the book also did the screen play, so it stayed true
Lord of the Rings, just cuz it was awesome
Peter Pan (the 2003 real-life version), once again, fairly true to the book
Hitchhiker's Guide to the Galaxy, absolutely freaking hilarious.

A Time to Kill. It was based on a John Grisham book and was done really well. Another one, also based on a John Grisham novel, was The Client.

I realize that many may disagree; however the stats would prove that GONE WITH THE WIND is the best movie that was based on a book.
