My English teacher had given us an assignment called "The Ways I Need to Study to Achieve Success."
Actually it should be 'has given us' because 'given' is past tense and by putting 'had' you will be written two past tense words and that isn't proper English.

Usually, titles are in italics (for typed) or underlined, and if you don't mind my meddling, I would change the sentence to either:
My English teacher assigned a paper titled The Ways I Need To Study To Achieve Success
or
Our English teacher gave an assignment about study habits, titled The Ways I Need....
and if that is not a true story and that's really not the name of a paper you had to write, then I would say it is a bit wordy and maybe should read Achieving Success Through Effective Study Habits.

It drives me crazy because every teacher is different! Some want quotation marks and some want you to underline. I really don't think they know what is correct, so just do what each teacher wants. Hand in your paper and the teacher will mark what they want in red pen. Then follow that format for the rest of the term, even if another teacher wants it differently.

Put quotation marks around the title of the assignment. Don't forget to put a comma before the quotation marks.

Assuming you're in the United States. For example, I think in the UK, it would be My English teacher had given us an assignment called "The Ways I Need to Study to Achieve Success".

iF YOU ARE USING THAT FOR THE TITLE OF YOUR eSSAY, IT CAN DEPEND ON THE FORMAT SHE WANTS IT IN, BUT USUALLY IT IS CENTERED AT THE TOP OF THE PAGE WITHOUT ANYTHING, JUST REGULAR. (sorry my caps lock was stuck.)
