Prayer = request. Judgment = decision, and remedy.

The phrase means asking for a decision, and then if the decision is in your favor, getting the remedy that you asked for.

"Prayer" is an archaic synonym for "request." A Prayer for judgment is nothing more than a request that the court enter a judgment.
