As another person posted, it really depends on the model. I have had some parts fall out of my Honda. like a special spacer that fit inside the hub. Also had a bearing (intact) fall out of its race, that was unusual in that they are normally hard to get out. There won't be a lot of parts falling out at once when you change a tire if that's what you are concerned about. Go slow and watch it as you separate the hub/wheel from the rear drive. good idea to place a newspaper or rag under the area to catch anything. Even better idea to change the tire at home for the first time rather then out in the country side.

Parts may fall out of your hand! The wheels can be a lot heavier than you think if you have only removed rear wheels from chain driven motorcycles. Most don't have any additional loose parts. If something falls out, it may be something broken! The big pain is getting everything to line back up when you reassemble and attempt to insert the axel. Make sure you have help at that point! Other than that, you should be pretty safe!

Always give year, make, engine size and model # when asking questions. All bikes are not created equal.
Jap bikes - nothing will fall out, that you won't notice. The drive flange, rubber dampeners and spacer can come off the wheel. They're so big you couldn't miss them. About the size of the flange and dampeners that a sprocket attaches to, on a bike with chain & sprockets.

No. You have the spacers outside of the hub, but nothing inside that will fall out. Remove the wheel carefully as there should be a rubber O around the splines.
