This is caused by evaporation more than anything, but yes - fish breath the O2 in from the water, and of course do ingest some (thus why there is urea in addition to fecal matter).

I don't think you have to worry about it to much. I'm sure your not the only one who ever swallowed fish tank water, besides I have saw cats and dogs drink out of them and nothing happened to them. Do you feel sick? If you do it is probably more in your head then anything else because you know you swallowed it and you know that fish use the bathroom in the water, which is gross and would make me sick to my stomach. Maybe you should get a hose and things that have a pump on it so you don't have to suck it to get it started. The medicene you use to treat the tank with should have instructions and warnings on the label. Read that and see what it say's, although if it says don't indjest don't panic, because it wasn't like you drank it straight from the bottle, it was dilluted with all the water in the fish tank. If you are still nervous and feel sick about it call your doctor, or get on line and go to Ask A Nurse and they will tell you what to do.

No, the water in the tank evaporates, just like water in a lake or even a puddle. Fish don't drink water - they don't need it for nutrients or to stay hydrated for that matter... it would be the equivalent of a human drink air....

Fresh water fish absorb water constantly through their gills. This water must be excreted constantly. (Salt water fish are constantly losing water through their gills, must swallow water regularlly, and excrete salt.) This doesn't result in a lost of water. That's due to evaporation.
