i had a similar problem, where my hands were dry and would crack and bleed, especially along the lines. I had some steroid cream prescribed which helped. Now, everday at least once i put on Eucerin cream, which is available anywhwere. It really makes a difference.

They sound as if they are chapped.
She needs to use Nutrogena until the cracks heal - possibly applying it before bed and then donning gloves available from the chemist which consist of plastic with breathing holes to retain the moisture. Daytimes she should smear it on the lint on a plaster and wear that. You can also buy plasters which are guaranteed to stay in place for a week even when showering.
Once the problem clears, a hand moisturiser should be applied 3-4 times daily as a preventative measure.

Neutrogena makes an excellent hand cream, they call it their Neutrogena Norwegian Formula Hand Cream. It is for extremely dry/chapped hands. Using this at night with the socks like other people said may help. Also every single time she washes her hands she should apply lotion or cream. Bed, Bath & Beyond (& maybe other stores) sell the gloves that you wear at night after you put on lotion or hand cream. Here is a link to see what the neutrogena handcream looks like . . .

When I lived on a farm my hands did this and I tried everything under the sun. I finally found relief by slathering my hands with vaseline and a very concentrated type hand lotion such as nivea and putting socks on my hands before I went to bed. It took about three days but my hands started healing and was able to just use lotion at night with the socks and they healed. She might want to try this and see if it works.
I feel for her, its very painful

pure shea butter might work, if not try a medicated lotion or cream. Aveeno works for me but in the winter i use shea butter (can be bought from Whole Foods) on my feet because they sometimes get really dry and tend to crack as well.

Apply large amount of vaseline on both hands at night and wear gloves made of cotton. Do it every single night... and then in the morning....wash hands with mild soap then apply mild lotion...repeat the same every night....
I can imagine that any skin diseases had been ruled out since she had seen doctors...condition like eczema...psoriasis... etc...

have her liver checked for disease....it could be a sign of a hereditary disease of the liver, or viral such as hepatitis c.
(hcv now has a 4 in 1 ratio of infection) your sister is 50 which puts her in an age group where HCV is high.

you can try eucerin's dry skin therapy creme...otherwise, she might have a condition (tho i dont know what its called, but i knew someone who had it all her life...her skin was so dry no matter waht kind of cream/lotion she used that it would bleed--especially at the knuckles and wherever the skin folds)
