There are drive line modifications like and external overdrive and if you have oversized tires you can also change the ring and pinon gears in the differential.
I Have a GMC 2500HD and with a realatively inexpensive kit from Banks which included exhaust, intake, and engine managememtn chip i am averaging about 20MPG but the best gain is pulling my 30 foot enclosed trailer i average about 17mpg.
