It's usually predetermined before you start the game. Normally, depending on how many folks are playing, it's set to when the dealer button goes around and is back at the original dealer, the blinds go up.

In tournament poker they play in whats called "rounds" or "levels"
Each round or leval has a pre-determined time limit 1/2 hr or 1hr for example but could be any pre-determined time limit.

Most home games I have played the blinds go up after each player is eliminated.

Clock. The clock is set to alarm. When the alarm goes off, the level goes up next hand. The speed of the level increases depends on the tournament. Better tournaments have a slow clock and small blind increases.

If its a tournament situation your are asking about, the blinds go up after a certain amount of time, or numbe rof hands. If it is a cash game, the blinds go up when the players agree to raise them, usualby majority vote.

It depends. Usually it's after a predetermined amount of time, although some online places do hands instead which I hate so I don't play at those sites anymore.
