It's to my knowledge a term of endearment. It's debatable who Really is "the 5th Beatle". Some say it's radio dj Murray the K (who promoted them heavily when they 1st came to NYC), some say it's producer George Martin & others say it was keyboardist Billy Preston. (played on songs off Let It Be & Abbey Road)

Ringo Star the drummer replaced the original drummer. They call that person replaced the fifth Beadle. Forget name. They might be calling person unlucky, second rate performer or has been.

Pete Best was the "fifth Beatle." He was the original drummer.
Stuart Suttcliff was a bass player who never really was i nterested. Besides, he was not a left-handed bass player.
