It is where you write out the source and then write a paragraph about the source. For example:

Astin, Alexander W. and Helen S. Astin. Leadership Reconsidered: Engaging Higher Education in Social Change. W.K. Kellogg Foundation, Michigan: 2000.

Astin and Astin introduce the Social Change theory of leadership development and apply it to an institution of higher education. This theory is based off of seven core values, the “7 C’s”, which include the individual values of consciousness of self, congruence, and commitment; the group values of collaboration, common purpose, and controversy with civility; and the societal value of citizenship. Believing that leadership is a process, not a position or title, the authors encourage that all students participate in a leadership development program. Specific attention is given to what student affairs professionals, academic affairs personnel, and administration could do to foster change and implement practices that could create more engaged leaders.

This was pulled right off of one of my papers. Just do something like this for all of your sources.

When you write the references, you include remarks with regard to how appropriate or relevant the reference is.

For example, if you cite in your reference list a web site document source but you know that there is another source that cites contrary contents, you may want to add "worth contrasting with [reference 2] due to differing perspectives or approaches".

Or, Author X, Publication Title, etc. Then add: "This view was subsequently debunked by Author Y in his publication entitled Publication Title, etc."

That's my take on it.
