It might be hard to avoid a negative reaction on her part cause people dont like critism but try this;
Tell her exactly what she did that hurt you but also try to understand her point of view and why she behaved the way she did. You could say something like "Look, I understand why you acted like this and you probably didnt mean to hurt me but I need you to know that it hurts me when you do this" I dont really know the situation but alot of times when people get hurt by someone, the someone may totally not have meant to, totally didnt realize it and dont want to hurt you. Let her know you understand that and just to be mindful of it in the future. You can do it in a really kind way. Usually, thing go well if you think of how the other person might feel first, even though it can be really hard to do. Good luck!

I agree that you just have to tell her. True friends should be able to discuss those types of issues. You may or may not hurt her feelings, however, I think that if you tell her in a way that you are not humilating her and tell her up front that your intentions are not to hurt her feelings but to express yours I would think that she would respect that.

IF you are really pissed, stop worrying about your friend's feelings and tell her straight up..bluntly....IF she is a friend, all will be fine. If not, then you need to move on anyway. good luck

Sometimes you have to be blatently obvious... other times you have to ease it on/.... think of how a counselor might talk to someone... maybe use some reverse phyc. make her wonder how you felt when she pissed you off... if shes totally oblivious? approach her.... say hey.. you said this or did that and it really upset me and we should talk about it....

Build - break - build

- or -

Compliment her - let her know how she messed up - compliment her.

In other words put a compliment before and after the remark you must make.

It is never what you say, but how you say it.

How is she pissing you off? That will depend on how you tell her.

If she's really your friend, you can tell her any way you like. It's okay if she gets mad. That's her right. She'll get over it.
