First , the doctor will give you a vaginal exam, and see if the Hymen is present. There are also other ways to tell physically in and around the vagina. About the orgasm... trust me, you will know! lol

The doc can tell if your hymen is intact and that generally means you are a virgin. But if it's broken that may or may not mean you had sex.
As for the orgasm thing, "how do you know" is not the right question. The question is," how do you prevent the neighbors knowing?"

doctor would never know whether you are still a virgin or not, except knowing whether you are having an intact hymen ! Only you yourself n the god know you are still virgin !
orgasm is the highest pleasure[ for most of the women ] from sexual intercourse[ love making ] n immediately after orgasm, you wouldn't wanted to have any more sex, you just wanted to stop n just soak in the great feeling like hugging your partner, whisper into his ear or just looking into his eyes full of emotion, give him a hot kiss as a gesture of thanks !

1. no one can really tell physically if you're a virgin. the old hymen story is a lie, because although the hymen exists, it is usually broken at a young age by normal physical activity. an non-intact hymen is useless in determining virginity, so have all the sex you want- just be sure to use protection.

the dr. will know (if you are a girl) if your hymen is in tact--which it will be if you haven't had sex or it won't be if you have. if you don't know if you have had an orgasm or not then chances are you haven't.
