She asked me and I do not know. But I knew we could get an answer here. Please help. I do not want her to have surgery.

Try a bigger cup size. That might help even the look out a little bit.

Being pregnant, everything is out of shape. Wait until the baby is born, and if she's going to nurse, then more than likely it won't be that noticeable. And even if she's not going to nurse, her breasts will go back to their normal and original shape and it probably won't be noticeable then either.

So yeah, tell her to get a bigger cup size and wait it out. If you try going to a breast surgeon, he/she will tell you the same thing.

Almost all Women's breasts are not the same size. Notably the left is usually larger than the right. Some women do have a noticeable difference, and speciality bras can be purchased for this. Cosmetic surgery is available also (if there's a really drastic difference), but should not be considered until after birth and nursing as the breasts will reshape somewhat afterwards.

If you're a good friend you could pull on the smaller one and make it bigger so they match. If you're a really good friend then there are other things you can do to the smaller one besides just pulling. And if you two want to earn some money, then videotape it and sell the tapes. Advertise innocuously with either youtube videos or a blogger blog with links back to your paid videos.

I would try getting padding from a bra store like victoria's secret and just wearing extra in the small one.

The only thing that comes to mind would be that if it really bothers her she might consider augmentation on the smaller breast or reduction of the larger one.
