It hasn't been proven, but scientists believe it was. During a huge collision with either an asteroid or another planet, a large chuck was knocked off (the moon) and just stayed in earth's gravitational orbit. Yes, I saw this on TLC channel.

Quite probably. That's why it always shows the same face to the Earth.
It is like saying that its revolution around the Earth happens in the same time as its revolution around its axis, which gives us the suspicion that Earth and Moon were two parts of the same body separated by some event at an early stage of their formation.

One of the supporting factors for the moon being formed from the Earth is that it has the same composition and density as the Earth's crust. This suggests that the close approach of a giant object dragged off some of earth's crust to form the moon.
The gravity of this material tugged it into spherical shape over time.

There is a theory that early in the Earth's history a Mars-sized objsuct collided with Earth and threw up debris.This debris mixed with rock from the collision object,and it clumped together to form the moon.although unproven,this theory would explain why some lunar rocks seem to be of Earth origin,and some of them don't seem to come from earth.
