I've been twice (American) and you can get a visa upon arrival at both Jakarta and Denpasar. They have a little window off to the side before immigration and you fill out a form with your info and pay a small fee.

I was last there in March and I bought my visa on arrival at the Jakarta airport with no problem. I carry a USA passport. Get your photograph taken in advance.

Depending on your Nationalities and puropose of visit, it varies. Also depending on the days you will stay there also changes the cost of the visa.
