"Do not resuscitate" is a request from the patient.
"Do not attempt resuscitatation" means it is physically impossible or would do more harm to the patient

There really isn't any difference -- it's just that different states have different names for them. They have to attempt resuscitation to resuscitate you, right? Don't confuse them with a living will (sometimes called a directive to physicians) which states that the person has made a decision not to be placed on life support if he/she is in a persistent vegetative state.
