This is different form Community to community. But if you have a health insurance and your trip ends up in the hospital, that insurance will be billed for the EMT service. In some cities like Burbank California, half the Fire department leaves the station when you call in with a hang nail and you will be billed for every vehicle they sent. If you have no money or can't pay they still must help you. Lots of Fire departments have "Fill the Boot" donation drives and that is where you can show your support and put a donation in the boot.
You could order Pizza for the fire station one night too.
In some cities the Fire department /EMT responds for the first aid part but private carrier EMT company ambulance will take you to the hospital and they are going to bill you or your health insurance

It depends on what city you are in. Most EMS services bill you or your insurance company if you are taken to the hospital. As far as the fire department goes...where I live tax dollars go towards that, but if you have the fire department come out for a false alarm then you get billed, but if it is for a real fire or emergency you are not billed.
