Something sentimental that money can't buy.
Write that person a card about your friendship or whatever relationship you have with them. I'm sure you can write some really nice things in it.
Try to give him/her a sentimental gift that reminds you of the relationship you have with them. Or something that they hinted that they liked. Maybe a gift card so they can buy new CDs or a gift card to the local movie theater.

Well that is where creativity comes into play. You need to do something more personal. Make a scrap book by hand or a picture frame detailed by hand with your picture in it. Or take him/her on a trip somewhere either one of you have been before and do like a treasure hunt or something. Main point. Be Creative.

You could make a photo book (or disk).
You could write a letter that describes::
>why he/she is so special to you
>your favorite memory of him/her
>how you first met
>your first impression of him/her
>the way you picture him/her when they are 20/30/40/50
A coupon book that gives him/her specific choices
Take this person on a picnic (no other friends)

i think even someone who has everything, still wants what anyone else would want on their birthday, and that would be to feel special and appreciated!! A birthday is a celebration of that person's birth. It doesn't need to be expensive or elaborate. Take them somewhere romantic (if its your significant other, and bring a bottle of champagne) And just let them know how much you care!

Seiously? I have a relative with the same affliction. I ended up doing a monthly subscription to a scrap booking club. I did Indi movie club, wine club, and movie clubs for others. It seems to go over really well!
