It's been 21yrs. I noticed this during a breast exam. no lumps, just a small amount of blood tinged colostrum. Could this be cancer?

Simple answer - yes, it could be a symptom of breast cancer. However, it may be something called ductectasia, inflammation of the ducts in the breast. But you really need to go and see a doctor and be examined, probably have a mammogram or ultrasound of the breast, depending on your age, and they will be able to tell you from that. Please, don't ignore this symptom, it could be a sign of something serious.

It probably isn't colostrum. There are different things that can cause disharge in the breast after years - the blood-tinged has less possible causes - injury, cancer or certain irritants in the ducts - best thing you can do is see your medical provider - the discharge can be swabed and sent in a Pap smear for cancer, it can also be evaluated under a microscope. There is also a proceedure called a ductal lavage that rinses discharge from some of the breast ducts for evaluation. Please get it checked out.
