They are talking about his shenanigans with Katie and his daughter out in public. Off-camera means what he has been doing after he punches out of work. Mostly he has been shamelessly wringing his personal life for publicity, talking badly about women who treat post-pregnancy depression with drugs and therapy, and just generally being over the top - like he could use some drugs and therapy his self.
