The term "secretary" could mean anything from an office assistant in some minor government agency, to a member of the Cabinet, like the Secretary of State or Secretary of Education.

If you mean the latter, then that person is the chief officers for that department (State Dept., Dept of Edu., etc.) and is responsible both for running that department, and for advising the president on such matters.
