You have to practice all the time. If you do not communicate often in English, you will not get any better at it. Reading about skills and doing exercises are not the same and will not help you as much.

Cultivate communications with native English speakers / writers.
Ask for constructive criticism so they will tell you when you make a faux pas.
Seek discussions in areas of interest to develop your vocabulary.

talk to people who speak fluent English. that way, by listening to people who communicate fairly well, you will improve your communicating skills. =] don't be afraid to make mistakes when talking!
