The foreskin should be able to be withdrawn without discomfort and certainly not pain. If this is causing concern, then certainly go to the doctor and explain the situation. It could be that the foreskin was not moved back gradually as a child until it became flexible. The doctor will respect what you say and advise you on the best course of action to take. It could involve circumcision, which today is a standard surgical procedure and risk free. A little discomfort now, or discomfort for life? Rip off? Hardly.

It is certainly possible - if you have a short frenulum - to be damaged by ordinary intimacy. If you tear your frenulum - the delicate and exquisitely sensitive connective tissue that pulls the foreskin back over the glans when you are flaccid - you can have a frenoplasty which sort of breaks the connection between the frenulum and the shaft skin so it won't get hurt again.
If you are in the US, it will be very hard to find a doctor who knows anything about the normal penis. Call a GP or urologist's office and ask: "does the doctor have a foreskin?" Keep dialing until you get a YES.

its not possible to rip or harm it if your practicing penis to vagina sex. as long as you got enough lubrication. if you dont think theres enough naturally then use some lube from a bottle or tube. anal sex is more likely to cause this type of problem. work the lube around the tip and get it under the foreskin so its easily pulled back during intercourse

Circumcision is unfortunately a standard procedure, but it is definitely not risk free. Tell the people whose penises were amputated, the infant who has brain damaged from herpes he got from an infected mohel, the young African men who died of infection, and the children who have bulbous masses where their penises should be that there are no complications. http://images.search.yahoo.com/search/images/view?back=http%3A%2F%2Fimages.search.yahoo.com%2Fsearch%2Fimages%3Fp%3Dcircumcision%26toggle%3D1%26ei%3DUTF-8%26fr%3DFP-tab-img-t-t500%26b%3D421&w=286&h=436&imgurl=www.stdservices.on.net%2Fimages%2Fstd%2Fbalanitis%2Fslide10.jpg&rurl=http%3A%2F%2Fen.wikipedia.org%2Fwiki%2FMedical_analysis_of_circumcision&size=22.9kB&name=slide10.jpg&p=circumcision&type=jpeg&no=439&tt=27,008&oid=b4b966bcb76a01a8&ei=UTF-8
As for your question, pull it back as far as you can without hurting yourself. If it even hurts slightly, stop doing it. If you feel pain during sex, you're being too rough. Ease up and it should get better.
