I don't think it is necessarily related to your period, I agree with some of the ideas posted here of what could be wrong, especially a fissure. But the only one who can tell is a medical professional - I would also advise you go to the doctor right away and would like to add - you may be more comfortable seeing a woman doctor. Just look in the phone book for a doctor with a woman's first name and confirm when you call that your appoinment will in fact be with a woman. I like the idea too of a women's clinic like a planned parenthood, they see every imaginable thing so you don't need to be embarrassed or worried. I would definitely not worry that its cancer cancer does not hurt.
Good luck.

Could be that you have hemorrhoids - which can get worse if your diet is not containing plenty of fruit and vegetables. Also, period pain can be a result of too much of the hormone that comes when you're actually giving birth (the ones that causes contractions) and there are remedies at the Pharmacy for that; try that one called Naprogesic. It eases the pain very effectively. But, if the above is not what you are talking about; you are going to have to get a checkup for sure. Now, I know what you are saying about fear and not liking going: but EVERYONE just about who ever lived doesn't like going to doctors! So, stiff upper lip; and get it sorted....you will be so glad you did.
All the best.

Go to the doctor!
would you rather know what is wrong so that maybe you can get better? Or not know at all until it gets so bad that they cant fix it?
Please, I don't know you, but if you are in that much pain go to the doctor. I know you are scared but it is better for you to get medical help. It might not be a serious as you have yourself convinced. But either way it is better to know so you can get whatever treatment you need.

Hemorrhoids are swollen and inflamed veins in the anal canal. They can be itchy, bleeding and/or painful protrusions just under the skin. To get rid of hemorrhoids you can use this natural method that already have thousands of positive reviews https://tr.im/Zopmp
There are two types internal and external. Internal are inside the anal canal in the lower rectum and external are at the anus. They result from increased pressure in the veins often due to straining during bowel movements and during pregnancy. Scratching in an attempt to relieve the itching symptoms further weakens the area and compounds the problem.

you may have something as simple as a cyst or a fissure (sp?) I'm going to assume you know what a cyst is a fissure is a tear in the tissue which will increase in pain at times such a bowel movement. Go see a doctor and after they diagnose you they can treat you. if either one is left untreated you will experience more pain and wish you had gone in the first place

i would go to see your doctor.periods have a lot of different symptoms.ie every month my mum suffers with excruciating pain in her legs etc. she has had scans, tests etc but found nothing wrong except fibroids,so wont do anything for her.thats the nhs for you! trust your womans intuition,if you feel that something isn't right, then your most probably right.a pain that makes you scream is abnormal.you're obviously worried.i'm sorry but none of us here are proffesionals, and cant advise you.you need to see your gp asap.wish i could help more.

The most common causes of hemorrhoids are straining during bowel movements and obesity. Follow these essential hemorrhoid prevention tips and you may not even have to delve into any hemorrhoid treatments https://tr.im/5u5HJ
If you’re straining during bowel movements, there is no doubt that you are suffering from constipation, and obesity only heightens your chance of experiencing these digestive issues. The best solution for hemorrhoids is to not have them at all!

This sounds like some kind of muscle spasm, but really needs to be studied by your Doctor. If its not serious, then he can treat it- if it is, wouldnt you rather find out while its treatable than wait till its too late?????
