1) As others have posted, if you already have an Internet connection in your home, then all you need is a wireless router to use the adapter at home. No extra charge.
2) Also as others have posted, if you are purchasing a wireless network adapter from a cellular company, then yes, you will pay an additional monthly fee.
3) You may also get a wireless adapter and use free wireless networks in many areas. Most Starbucks have free wireless available, even some grocery stores where I live have free wireless. there are many "hot spots" all over the world that offer free access.
So to sum up, it really depends on what type of card you are buy (cellular adapter or 802.11x) and where you want to use it.

Not to use the card. It's just like plugging your computer into your cable modem or DSL. You will still need to pay your internet service provider for service to your house (unless a neighbor has a WiFi network that you can access).

If you are using it within your home to connect wirelessly (which means you have wireless router and high speed internet connection) then no. But if you are buying it to use on cellular network like sprint then yes.
