SO that when a doctor is writing in the charts another health care worker will not be confused as to what time to give a medicine or treatment. We all know how a doctor (and other people) writes. But, If a med is prescribed at 4 hour intervals and a nurse documented that she gave it at 4a-8a-12p-4p-8p-12am. it is not as confusing if they were to say given at 0400-0800-1200-1600 2000-0000.when looking at the chart a person can over look the a and p. And sometimes people don't know am from pm. another reason is for purposes if it went to court a lawyer can't well did you get confused to the am or pm times. Short and simply it is a CYA thing.
