The cashiers at the stores I shop in enforce the 10 items or less rule, no matter what. "I'm sorry sir. You can't bring those items through this check-out. This line is for 10 items or less. Next!" If the person argues, the manager is called, and he gets to vent at the manager, while the express line moves quickly along as it's designed to.
If a customer is terribly irate at this, I have seen the manager ask one of the women cashiers (who may be off re-shelving abandoned products) to open her cash for this one person, or for 1/2 hour to allow the crush to ease...I've seen it done both ways.
What gets me upset is the person who has one or two items, and expects me to let him in front of me at the checkout that takes full loads, when I have been patiently waiting in line for 15 minutes to get that far. I just tell them, "I'm sorry, but I've been patiently waiting to get up here where I am. I think you should think about doing the same in the express check-out."

Sometimes you just have to be patient. I have been in a couple of those lines where the person didn't understand the language of the sign posted. It bothers me a little bit to wait as I am very impatient to begin with but..... life won't end if you end up being patient. Take the high road and let it go.
You can always complain to the manager to tell the cashiers to warn these people.

That happened to me the other day! I went in for tampons and ibuprofen, and it took me 45 minutes. The lady in front of me had like 50 items, and coupons for the items. It sucks!

I start counting loudly when they put their stuff on the conveyor. When I get to 10, I start with "1 over, 2 over, 3 over..." they scowl at avoid me, but I always turn to the person behind me and smirk.

Yes it makes me mad, but what's worse is when they don't open more than one regular line and one express line, or any of the self check-out counters. KROGER does that a bunch.
