I know I worked at one that will remain nameless years ago, and if someone who was preparing the food or one of the waitresses got pissed off, something really bad usually happened to the @ssholes food.
The worst thing I ever saw, a cook peed in someones milkshake.

i've worked as a server for over 10 years...i've seen some nasty stuff. i've never thought that they did anything to my food, personally. you have to know how to complain the "right way." have a legitimate reason, (steak well done when you ordered it med-rare), be nice, say please, smile, and say thank you. don't curse at your server, raise your voice, or belittle your server in any way. don't ask for a manager unless absolutely necessary...a server can fix most food issues without the manager to begin with.
never seen anyone pee in food, but i have seen steaks and burgers dropped on the floor and then picked up and thrown back on the grill....and i saw a cook scoop rice out of the pile in the trash can and serve it. i've seen steak knives scraped against the sole of a shoe and then put under the steak. visine in coffee...gives you diarrhea. i could go on...
when you go into a restaurant right before they close, bad things can happen. i won't go in even 45 minutes before close...i'm paranoid and sympathetic to them.

My husband's immature ex has a glamorous job as a waitress at a local rest. I am sure her and her reject friends have made our food a little "tastier" before. We always make sure to tell our waitress in an over the top kinda way, how incredibly good our food was.

A spider in an ice cube. AFTER I'd already drank most of the soda. Didn't see the spider before because it was the same color as the soda. Yeah, I puked.

Gross!
And yes, I have thought that before.
I try not to piss anyone off in a restaurant EVER. I would like my food without anybody's bodily functions in it, please
