Hi. Because if it did it would have to be a parabola (paraboloid actually) in the case of a mirror. Also there is chromatic aberration which causes light of different frequencies to focus at different points in the case of a lens. (You did not rule out a mirror.)

To AN exact "point focus"? Well, one reason would be if the lens were of inferior quality and craftsmanship - sending the rays askew. Another reason would be that there is an obstruction between the lens and the focal point such that there is no room for the rays to intersect.
