No, visual imagery is used to enhance the senses where as hallucination is the addition of items/people into a scenario thus altering the minds perception with some excessively negative effects once the hallucinatory drugs have passed through your system (LSD/acid traces stay in your spinal column which when stimulated will evoke flash backs) resulting in a change in your brains neural functioning. The truth is visual imagery is used in writing to enhance the descriptive powers of the scene and hallucinations for the most part are associated with drug use or a specific psychological disorder that requires a differential diagnoses with a subsequent treatment plan.

No, mostly because I haven't been in a mental state that allows visual imagery to be physically manifested as an image. It has always remained an idea.

For it to be an hallicination, there would have to be something going on in the brain to kick the visual cortex on. And whatever did this (drug or mental illness) probably isn't going to just make real visual imagery, but all manner of odd things.

Yes it can happen to anyone, even if you haven't used acid. It can happen if you are tired, or sick. I can see dark figure out of the corner of my eye when I am tired sometimes it even makes me jump.
